CCM and APCM Candidate Identification for Clinician Review
Find patients who may qualify for CCM, PCM, APCM and RPM, ready for your clinicians to review.
What does Care Management Intelligence do?
Care Management Intelligence finds patients who may qualify for chronic care management, principal care management, advanced primary care management or remote monitoring but are not enrolled. It reads problem lists, visit history and coverage, checks for programs already billed, and sends each candidate to a clinician for review. Nothing is enrolled without clinician approval and patient consent.

What it finds
- Patients with two or more chronic conditions and no CCM enrollment
- Single complex conditions that may suit PCM
- Primary care panels that may suit APCM
- Heart failure and hypertension patients who may suit RPM
- Program overlap that would block billing
Data it uses
| Source | What it tells Nuvory |
|---|---|
| Condition | Problem list and encounter diagnoses |
| Encounter | Recent visits and provider |
| Coverage | Payer and plan type |
| EpisodeOfCare | Existing program enrollment |
How it works
- Step 1Read problem lists, visit history and coverage
- Step 2Look for qualifying conditions with no current enrollment
- Step 3Check for overlap with care management already billed
- Step 4Send to a clinician; nothing is enrolled without approval and consent
What can you measure with Care Management Intelligence?
Each figure is tracked against your own baseline from two years of history, by location and service line. Care Management Intelligence is included in the Growth and Enterprise plans. Compare plans.

Questions, answered
More in our FAQ, or ask our team.
Is Nuvory an RPM or CCM platform?
No. Nuvory finds patients who may be eligible and routes them to clinical review. Enrolled patients are cared for by your own team or by the HealthArc care team on the HealthArc platform.
Who decides whether a patient is eligible?
A clinician at your practice. Nuvory shows the signals it found, such as conditions, recent visits and coverage. The clinician approves the patient for outreach or marks them not eligible.
Is patient consent captured?
Yes. After a clinician approves, an MA or clinician records the patient's consent before enrollment, as CMS requires for these programs.
