Elation Health Revenue Intelligence for Referrals Out and Care Plans
Independent and value-based primary care practices on Elation send patients to specialists and keep them on long care plans. Nuvory is designed to show which referrals never closed and who drifted from the plan.
What is Elation Health revenue intelligence?
Elation Health revenue intelligence is using the patient, appointment, encounter, referral and condition data in Elation EHR to find referrals you sent with no visit recorded, patients overdue on their care plan and potential care-management candidates, then ranking who to contact first. Nuvory, healthcare revenue leakage intelligence from HealthArc, is designed to work with Elation EHR data through FHIR R4 APIs, with bulk export where enabled.
Why do referrals out matter so much on Elation?
Because a primary care practice is accountable for patients it sends elsewhere. A referral to a specialist that never results in a visit is a care gap the practice owns, and in value-based arrangements it can show up in quality results. Elation's FHIR data includes referrals, so referrals you send can be listed until a visit or consult note is recorded where your practice files it, or staff mark them done elsewhere. Where referred patients actually received care is a Network Intelligence view on Enterprise. For the method, read our guide to closed-loop referral management.
- Referrals out, not closed Referrals you sent with no visit or consult note recorded, where your practice files it and this is configured during setup.
- Care plans drifting Chronic and preventive follow-ups past due with nothing booked.
- Care-management candidates Patients who may be potentially eligible for CCM, PCM, APCM or RPM, routed to clinician review.

- Confirm referral dataHealthArc checks how your practice records referrals and which resources are available.
- Approve accessYour Elation administrator approves API access.
- Load and baselineAbout two years of history is loaded and measured.
- Clinician approvalA physician or nurse lead reviews real patients and approves each rule.
- Close items as care completesNightly runs remove patients once the visit is recorded or staff mark it done elsewhere.
Follow up on the referrals you send
Care management, scheduling and opportunity modules are on Growth at $299 per provider per month; recalls alone start on Starter. Network Intelligence, which shows where referred patients went, is on Enterprise.
How is this different from Practice Fusion?
The data. Elation's FHIR data includes referrals, so referrals you send can be scoped from the start where your practice records them. On Practice Fusion, referral data is confirmed case by case and many practices start with recalls only.
For primary care practices, care management intelligence surfaces candidates for clinician review; read more on finding eligible care-management patients. Data is encrypted, isolated per practice and covered by a business associate agreement; see security.

EHR reports describe records one type at a time. Revenue leakage intelligence matches referrals, orders and recalls to the visits and procedures that should follow, and checks whether care was completed. Here is where each fits.

Closed-loop referral management tracks each referral from creation to completed care, then returns the outcome to the referring provider. Here is how the loop works, where it breaks and what to ask when evaluating technology.
